This form is to request to become an authorized In-House trainer for the Division of Liquor Control’s In-House Training Program. All applicants must have successfully completed a DLL In Person seminar or the DLL Online Seminar. After you have completed the form, a representative of the Office of Education will contact you with further information.
This form can also be used to renew your trainer certification or if you have switched employers.
IMPORTANT: This form only provides us with contact information for anyone wishing to become a certified In-House Trainer. It does not certify you as an In-House Trainer. You must also currently have a valid DLC certification (1st class, 2nd class, etc.) and must register for and attend a FREE "Train the Trainer" virtual seminar. Please visit the Seminars section of our WEB page and choose Classroom/Virtual Seminars after submitting this form to register.
