This form is intended for use by Retail Liquor Agencies and State drivers in the event of an accident during a delivery or other visit. If there is damage to a Retail Liquor Agency's property during a delivery by the State, please fill out the following form and attach pictures of the damage. Agents should attach an estimate of damage repairs.
